Bad Breath: Symptoms, Causes and Remedies

Whether you call it bad breath or halitosis, it’s an unpleasant condition that’s cause for embarrassment. Some people with bad breath aren’t even aware there’s a problem. If you’re concerned about bad breath, see your dentist. He or she can help identify the cause and, if it’s due to an oral condition, develop a treatment plan to help eliminate it.


What you eat affects the air you exhale. Certain foods, such as garlic and onions, contribute to objectionable breath odor. Once the food is absorbed into the bloodstream, it is transferred to the lungs, where it is expelled. Brushing, flossing and mouthwash will only mask the odor temporarily. Odors continue until the body eliminates the food. Dieters may develop unpleasant breath from infrequent eating.


If you don't brush and floss daily, particles of food remain in the mouth, collecting bacteria, which can cause bad breath. Food that collects between the teeth, on the tongue and around the gums can rot, leaving an unpleasant odor.



Bad breath can also be caused by dry mouth (xerostomia), which occurs when the flow of saliva decreases. Saliva is necessary to cleanse the mouth and remove particles that may cause odor. Dry mouth may be caused by various medications, salivary gland problems or continuously breathing through the mouth. If you suffer from dry mouth, your dentist may prescribe anartificial saliva, or suggest using sugarless candy and increasing your fluid intake.


Tobacco products cause bad breath. If you use tobacco, ask your dentist for tips on kicking the habit.
Bad breath may be the sign of a medical disorder, such as a local infection in the respiratory tract, chronic sinusitis, postnasal drip, chronic bronchitis, diabetes, gastrointestinal disturbance, liver or kidney ailment. If your dentist determines that your mouth is healthy, you may be referred to your family doctor or a specialist to determine the cause of bad breath.


Maintaining good oral health is essential to reducing bad breath. Schedule regular dental visits for a professional cleaning and checkup. If you think you have constant bad breath, keep a log of the foods you eat and make a list of medications you take. Some medications may play a role in creating mouth odors. Let your dentist know if you've had any surgery or illness since your last appointment.

How do I find a dentist who treats bad breath?


Dentists are taught in dental school about the oral causes of bad breath (halitosis). If you have bad breath, you may want to start by talking to your general dentist about treatment options. You can also contact your state dental association to see what resources are available in your area of the country.

What is the treatment for bad breath?


If your dentist determines that your mouth is healthy and that the odor is not of oral origin, you may be referred to your family physician or to a specialist to determine the cause of the odor and for treatment. Of course, if the odor is of oral origin, as it is in the majority of cases, your dentist can treat the cause of the problem.

If the odor is due to periodontal (gum) disease, your general dentist can either treat the disease or refer you to a periodontist, a specialist in treating gum tissues. Periodontal (gum) disease can cause gum tissues to pull away from the teeth and form pockets. When these pockets are deep, only a professional periodontal cleaning can remove the bacteria and plaque that accumulate. Sometimes more extensive treatment is necessary.

If you have extensive build-up of plaque, an invisible layer of bacteria, your dentist may recommend using a special antimicrobial mouthrinse. Your dentist may also recommend that when you brush your teeth, you also brush your tongue to remove excess plaque.

How Effective Are "Halitosis Kits" and Other Products That Claim to Treat Bad Breath?


The ADA cannot vouch for the claims the manufacturers of halitosis kits make. If you are concerned about their safety and effectiveness, you can ask your dentist if the products in the kit will be useful for you.

Many antiseptic mouthrinse products, however, have been accepted by the ADA for their therapeutic benefits in reducing plaque and gingivitis and also have breath freshening properties. Instead of simply masking breath odor, these products have been demonstrated to kill the germs that cause bad breath. You may wish to ask your dentist about trying some of these products.

Proper Flossing Techniques


The ADA recommends the following for good oral hygiene:   
  • Brush your teeth twice a day with an ADA-accepted fluoride toothpaste. Replace your toothbrush every three or four months, or sooner if the bristles are frayed. A worn toothbrush won't do a good job of cleaning your teeth.
  • Clean between teeth daily with floss or an interdental cleaner. Tooth decay–causing bacteria still linger between teeth where toothbrush bristles can’t reach. This helps remove the sticky film on teeth called plaque and food particles from between the teeth and under the gum line. 
  • Eat a balanced diet and limit between-meal snacks.
  • Visit your dentist regularly for professional cleanings and oral exams.  
Antimicrobial mouth rinses and toothpastes reduce the bacterial count and stop bacterial activity in dental plaque, which can cause gingivitis, an early, reversible form of periodontal (gum) disease. ADA-Accepted antimicrobial mouth rinses and toothpastes have substantiated these claims by demonstrating significant reductions in plaque and gingivitis. Fluoride mouth rinses help reduce and prevent tooth decay. Clinical studies have demonstrated that use of a fluoride mouth rinse and fluoride toothpaste can provide extra protection against tooth decay over that provided by fluoride toothpaste alone. Fluoride mouth rinse is not recommended for children age six or younger because they may swallow the rinse. Consumers should always check the manufacturer’s label for precautions and age recommendations and talk with their dentist about the use of fluoride mouth rinse.

Talk to your dentist about what types of oral care products will be most effective for you. The ADA Seal on a product is your assurance that it has met ADA criteria for safety and effectiveness. Look for the ADA Seal on fluoride toothpaste, toothbrushes, floss, interdental cleaners, oral irrigators, mouth rinses and other oral hygiene products.  

What is plaque?

Plaque is a sticky film of bacteria that forms on teeth and gums. Following a meal or snack, the bacteria in plaque release acids that attack tooth enamel. Repeated attacks can cause the enamel to weaken, eventually causing tooth decay. Many of the foods we eat cause plaque bacteria to produce acids. If you snack often, you could be having acid attacks all day long. Plaque that is not removed with thorough daily brushing and cleaning between teeth can eventually harden into calculus or tartar.
Plaque also produces substances that irritate the gums, making them red, tender or bleed easily. After a while, gums may pull away from the teeth. Pockets form and fill with more bacteria and pus. If the gums are not treated, the bone around the teeth can be destroyed. The teeth may become loose or have to be removed. In fact, periodontal (gum) disease is a main cause of tooth loss in adults.
One way to prevent tooth decay and periodontal (gum) disease is by eating a balanced diet and limiting the number of between-meal snacks. If you need a snack, choose nutritious foods such as raw vegetables, plain yogurt, cheese or a piece of fruit.

What are some tips for daily oral care?

The best way to remove decay-causing plaque is by brushing and cleaning between your teeth every day. Brushing removes plaque from the tooth surfaces.
Brush your teeth twice a day, with a soft-bristled brush. The size and shape of your brush should fit your mouth, allowing you to reach all areas easily. Use a toothpaste that contains fluoride, which helps protect your teeth from decay. When choosing any dental product, look for the American Dental Association Seal of Acceptance, an important symbol of a dental product's safety and effectiveness.

Cleaning between the teeth once a day with floss or interdental cleaners removes plaque from between the teeth, areas where the toothbrush can't reach. It is essential in preventing periodontal (gum) disease.

By taking care of your teeth, eating a balanced diet and visiting your dentist regularly, you can have healthy teeth and an attractive smile your entire life. Follow these tips to keep your teeth and mouth clean.  

How do I brush my teeth?

  • Proper tooth brushing technique.Place your toothbrush at a 45-degree angle against the gums. 
  • Move the brush back and forth gently in short (tooth-wide) strokes.
  • Brush the outer tooth surfaces, the inner tooth surfaces, and the chewing surfaces of the teeth.
  • Use the "toe" of the brush to clean the inside surfaces of the front teeth, using a gentle up-and-down stroke.
  • Brush your tongue to remove bacteria and freshen your breath.

How do I floss my teeth?

  • Break off about 18 inches of floss and wind most of it around one of your middle fingers. Wind the remaining floss around the same finger of the opposite hand. This finger will take up the floss as it becomes dirty. Hold the floss tightly between your thumbs and forefingers. 
  • Guide the floss between your teeth using a gentle rubbing motion. Never snap the floss into the gums.
  • When the floss reaches the gum line, curve it into a C shape against one tooth. Gently slide it into the space between the gum and the tooth.  
  • Hold the floss tightly against the tooth. Gently rub the side of the tooth, moving the floss away from the gum with up and down motions.
  • Repeat this method on the rest of your teeth.
  • Don't forget the back side of your last tooth.  
People who have difficulty handling dental floss may prefer to use another kind of interdental cleaner. These aids include special brushes, picks or sticks. If you use interdental cleaners, ask your dentist about how to use them properly, to avoid injuring your gums.

Jaw Pain: Diagnosis and Treatment

Many adults suffer from chronic facial pain. Some common symptoms include pain in or around the ear, tenderness of the jaw, clicking or popping noises when opening the mouth, or headaches and neck pain.
Two joints and several jaw muscles make it possible to open and close the mouth. They work together when you chew, speak or swallow. They include muscles and ligaments, as well as the jaw bone--the mandible (lower jaw) with two joints called the temporomandibular joints or “TMJ.”

The TM joints are among the most complex joints in the body. Located on each side of the head, they work together to make many different movements, including a combination of rotating and gliding actions used when chewing and speaking.

Several muscles also help open and close the mouth. They control the lower jaw (mandible) as it moves forward, backward, and side-to-side. Both TM joints are involved in these movements. Each TM joint has a disc between the ball and socket (see diagram). The disc cushions the load while enabling the jaw to open widely and rotate or glide. Any problem that prevents this complex system of muscles, ligaments, discs and bones from working properly may result in a painful TMJ disorder.

Diagnosis & Treatment

A dentist can help identify the source of the pain with a thorough exam and appropriate x-rays. Often, the pain may be from a sinus problem, a toothache or an early stage of periodontal disease. But for some types of pain, the cause is not easily diagnosed. The pain may be related to the facial muscles, the jaw or the TM joint.

Some TM problems result from arthritis, dislocation or injury. All of these conditions can cause pain and dysfunction. Muscles that move the joints are also subject to injury and disease. Injuries to the jaw, head or neck, and diseases such as arthritis, might cause some TM problems. Other factors relating to the way the upper and lower teeth fit together (the bite) may cause some types of TM disorders. Stress and teeth grinding are also considered as possible factors. 

Diagnosis is an important step before treatment. Part of the dental examination includes checking the joints and muscles for tenderness, clicking, popping or difficulty moving. Your complete medical history may be reviewed, so it’s important to keep your dental office records up-to-date. Your dentist may take x-rays and may make a "cast" of your teeth to see how the upper and lower teeth fit together. Your dentist may also request specialized x-rays for the TM joints. Depending on the diagnosis, the dentist may refer you to a physician or another dentist.

There are several treatments for TMJ disorders. They may include stress-reducing exercises, wearing a mouth protector to prevent teeth grinding, orthodontic treatment, medication or surgery. Treatment may involve a series of steps beginning with the most conservative options. In many cases, only minor, relatively non-invasive treatment may be needed to help reduce the pain.
